New DSCP form issued by MassDEP

Recently, MassDEP revised Title 5 Form 2A – the Disposal System Construction Permit.  This new version of the form is located on DEP’s website and is available for use immediately by local and regional health departments across Massachusetts.  Here is a link to the revised form Form 2A Disposal System Construction Permit 08.17.2026-a11y.pdf

The revised form contains more clarifying information about the work which is proposed to be implemented to repair or install an onsite system.  Additionally, the form complies with accessibility requirements to be usable by all viewers.

A Disposal System Construction Permit (DSCP) is the approval issued by a local or regional health department to a licensed System Installer to construct, replace or repair an onsite wastewater system or its individual components. If the work is for installation of a complete onsite system meeting either the Upgrade or New Construction standards in Title 5 then the DSCP should reference the design plan and supporting material that was approved in the Application for a Disposal System Construction Permit (Form 1A).
